The GKV-Beitragssatzstabilisierungsgesetz Act to Stabilise Statutory Health Insurance Contribution Rates, GKV-BStabG) has been in force since the 30th of July 2026. For the provision of medical aids, it becomes tangible from the 1st of January 2027: anyone calculating contracts, drafting quotes, or planning a listing in the list of aids from that point on will be working with a new reimbursement logic – and considerably less room to manoeuvre than before.
The most striking change is a flat three per cent discount on all provisions under § 33 SGB Vthat begin between the 1st of January 2027 and the 31st of December 2028. It comes on top of a permanent cap on reimbursement increases and a newly recalibrated fixed-amount system. Taken together, the Act reshapes the financial calculation basis for the entire sector – and does so, as the industry associations unanimously point out, in a segment that, according to the Finanzkommission Gesundheit (FKG), is not actually one of the cost drivers of the GKV-Spitzenverband (GKV) .
Three Per Cent Less on Every Provision Starting in 2027 or 2028
The discount does not apply to existing reimbursement as a whole, but to each individual provision that begins within the stated period. What matters is the start of the care activity relating to the individual insured person – not the date the contract was signed or the invoice issued. For medical supply retailers, orthopaedic technology businesses and manufacturers with their own care provision, this means the discount applies directly to every new provision, regardless of how an existing contract was calculated. With a total volume of around €12 billion in the medical aids sector, the government expects annual savings of roughly €360 million – money that will be missing elsewhere in providers' cost calculations.
Less Room to Manoeuvre in Price Negotiations
For contracts under § 127 SGB V , the standard wage rate (Grundlohnrate) becomes the fixed ceiling for reimbursement increases going forward. Full refinancing of collectively agreed wage increases beyond that rate is no longer possible. From 2027 to 2029, an additional one-percentage-point reduction applies, meaning real reimbursement increases of around 3% a year are likely to be the practical limit. Anyone looking to factor staff costs, energy prices or material costs into negotiations with the health insurance funds will have less room to argue their case than before. The standard wage rate minus one percentage point is, incidentally, not a special sacrifice asked only of the medical aids sector – it is part of a savings mechanism that applies across sectors.
Fixed Amounts Will Track Your Own Prices More Closely
For manufacturers with products listed in the German Medical Technical Aids Register Hilfsmittelverzeichnis (HMV), the overhaul of the fixed-amount system (§ 36 SGB V) is the change with the greatest strategic weight. The GKV-Spitzenverband gains extended data access rights and can request turnover and sales figures, calculation surcharges and hourly billing rates directly from manufacturers and providers. The calculation basis for new fixed amounts becomes a manufacturer price that may not exceed the upper price line of the lowest third, weighted by volumes sold – so anyone currently positioned in the upper price segment of their product group should expect noticeable pressure to adjust. A transition period allows contract prices to deviate by up to 15% from previous fixed amounts; after that, fixed amounts will be reviewed every three years. From 2029, the government calculates annual savings of around €500 million as a result.
Higher Co-Payments for Insured Persons
Co-payments, unchanged since 2004, are rising by around 50% (the minimum from €5 to €7.50, the maximum from €10 to €15) and will subsequently be indexed to the standard wage rate. For providers with direct patient contact, this often means more explanation and more collection effort in practice.
What the Industry Associations Are Saying
Under Section 139 of Book V of the German Social Code (SGB V) the BVMed forderte im Gesetzgebungsverfahren die ersatzlose Streichung des pauschalen Abschlags , proposing instead that existing reimbursement prices simply be frozen for 2027 and 2028. BVMed managing director Dr Marc-Pierre Möll summed up the criticism: "Stabilising contribution rates must not come one-sidedly at the expense of care, innovation and quality – it has to address structural causes and be designed sustainably." Der Bundesinnungsverband für Orthopädie-Technik (BIV-OT) put it more blunty : its president, Alf Reuter, said the discount would "finally break the back" of businesses providing local, community-based care, pointing to a skills shortage of just twelve applicants for every 100 vacant positions.
What Is Worth Checking Now
There's still time before the year-end, but it's worth using. It makes sense to review your own listing situation in the German Medical Technical Aids Register Hilfsmittelverzeichnis (HMV): are your products assigned to the correct product groups, does the documentation meet current quality requirements, and is it foreseeable whether an upcoming revision will affect your product group? Exactly how this will later affect individual reimbursement prices is a decision for the GKV-Spitzenverband alone – that's not something we can or would want to predict.
The Act also points to something more fundamental: how much a single payer can determine your margin when most of your revenue comes from one market. Manufacturers relying exclusively on German statutory health insurance reimbursement carry the full weight of regulatory changes like this one.
For manufacturers with products intended for the European or global market, it can therefore also be worth looking at other markets.
